NEW PATIENT PAPERWORK

New patients are asked to fill out information on the patient portal before their appointment. If you are unable to fill out forms online for any reason please arrive 15 minutes early to your appointment to use our system. The normal check-in process takes approximately 5 minutes once the portal is complete. If you are unable to access the portal online and you do not arrive early it will use up valuable appointment time.

CANCELLATION POLICY

Cancellation or rescheduling less than 24 hours before a scheduled appointment or a no-show of a scheduled appointment will result in a $50 cancellation fee. This fee buffers staffing costs incurred by untimely cancellations.

LATE ARRIVAL

If you are late for an appointment you will still be seen but your appointment will end at it’s scheduled time, in fairness to the next family and to our staff.

PARKING

2 hour street parking is available. During normal business hours and under normal conditions parking will likely be available within 1-3 blocks of our building. Please allow for enough time to find parking and walk, we recommend adding 10 minutes to your departure time. Special parking for women in labor is available.

BUILDING ACCESSIBILITY

The P Street entrance has 12 steps from the sidewalk to the front door. Large strollers are not recommended but if you need help please call and a staff member will assist you. Handicapped access is available through the rear of the building.

COVID POLICY

The Covid pandemic is rapidly changing so we reserve the right to modify our policy as needed. Mask requirements will expire 4/2/23. No person may enter the building who is sick with any viral illness, for the safety of the babies we care for.
